On-The-Job Injury / Illness Program

The OJI/Illness Program is responsible for coordination of benefits payable (medical expenses and lost wages) to any employee who may have an injury/illness sustained in the course of, and arising out of, employment at UAB.

For needle sticks and/or other exposures (including blood or body fluids), call the Rapid Response Team at 934-3675. Do not go to The Workplace or Emergency Department for a needle stick or exposure injury.

If you have an injury requiring medical treatment that would cause a bill to be generated, you MUST fill out an OJI Application for Benefits form as well as a Release of Information form. The Trend tracker Incident Report must also be completed before any bills will be paid.

Transporting Injured Employees
Beginning Tuesday, January 3, 2006, the On-the-Job Injury/Illness Program will no longer use ‘Yellow Cab’ (Greater Birmingham Transportation Service) to transport injured employees to The Workplace for treatment. We will instead use UAB Campus Ride at 975-TAXI (975-8294) to provide transportation to and from The Workplace for the initial visit. As we recently communicated, the new location of The Workplace is the old HealthSouth Building located at 1201 11th Avenue South, Suite 100. Their telephone number remains the same (933-5300).

When you call Campus Ride to arrange transportation, you must inform the dispatcher that you are calling to schedule a ride for an injured employee to be taken to The Workplace. The account number to provide is 7801110.
